Reason
A fault in Lot 583 of CoaguChek PT test strips, packs
of 12, which may cause an inaccurate INR result. The fault is in the
'easy open' notch on the individual foil pouches containing the
strips. In faulty pouches, the notch cuts into the seal of the
pouch, which can result in a damaged test strip. There is a small
risk that, as a result, patients using these strips may receive
inadequate anti-coagulant therapy, and therefore be at increased
risk of thrombosis.
You will find the lot
number on the top of the outer pack, after the word 'Lot'. It is
also stamped on each individual foil pouch.
